CASE OF THE DAY

A 60-year-old man presents to his primary care physician (PCP) with a 5-year history of shortness of breath and chest pain with exertion. His medical history includes chronic obstructive pulmonary disease, hyperlipidemia, hypertension, and smoking two packs of cigarettes and consuming five to six beers daily. His PCP refers him for a nuclear stress test, which reveals a moderate zone of lateral and inferior wall ischemia, a reduced left ventricular ejection fraction of 40%, and global hypokinesis.
